John A. “Jack” Smith, 61 of New Berlin, died peacefully at home on Sunday, March 13th. Jack was born in Red Bank, NJ, on April 8, 1954. He lived in New Jersey before moving to Oxford in 1983. In February 2015, Jack moved to a family care home in New Berlin, where he attended the New Berlin dayhab program.
Jack was predeceased by his father, William Smith, and his older brother, David Smith. Jack is survived by his mother, Agnes Smith, of New Berlin as well as five cousins: Ann Mahoney of Charlotte, NC; Lois Hammond of Leonardo, NJ; Martha and Susan Mahoney of Whiting, NJ; and Carol Gisinger of Beachwood, NJ. Jack is also survived by his wonderful caregivers Dale and Bernice Brownell.
A graveside service will be held at 1 PM on Saturday, April 23rd, at Riverview Cemetery in Oxford. Rev. Norman Holtz will officiate. Memorial donations may be made to a charity of your choice. Arrangements are being handled by the Behe Funeral Home, 21 Main St., Oxford, NY 13830.
