Chase Memorial staff honored for outstanding quality of care Chase Memorial Nursing Home staff were recognized on Wednesday, November 12, whose work led the facility to earning a 4-star Quality Measures rating from the Centers for Medicare and Medicaid Services.

NEW BERLIN - Chase Memorial Nursing Home & Rehab held a community Town Hall Celebration on Wednesday, November 12, spotlighting the staff whose work led the facility to earn a 4-star Quality Measures rating from the Centers for Medicare and Medicaid Services.

The designation reflects improvements in clinical performance and care coordination, marking one of the strongest quality scores the New Berlin facility has achieved in recent years.

During the event, Chase Executive Director Patti Hitchcock received a commendation from New York State Assemblyman Joe Angelino, who presented the honor on behalf of the state in recognition of the team’s efforts. Jason Santiago, President and CEO of Good Shepherd Communities, also addressed the crowd and credited staff for the their quality care achievements.

Leaders said the 4-star Quality Measures score represents the direct work of frontline staff, highlighting gains in areas such as reducing hospital readmissions, strengthening care coordination, and managing key clinical risks. Chase Memorial also recently advanced to an overall 3-star facility rating, an improvement supported by ongoing staff training and targeted quality initiatives.

Hitchcock told attendees she was proud to see the nursing home’s progress recognized publicly and emphasized that the achievement reflects daily effort from employees who have long-standing ties to residents and families in the New Berlin community. She said the team remains focused on continued improvements as the facility and offering the best care possible.

Chase Memorial Nursing Home & Rehab is part of Good Shepherd Communities, a nonprofit senior-care network that provides services ranging from independent and assisted living to long-term care and skilled nursing across several campuses.
